請完整填寫此表單，此為參加所有 JGL 賽事前須完成的一次性註冊。
Please complete this form to be added to the JGL Master Roster.
This is a one-time registration required before signing up for any JGL event.
1. Be sure to enter your full legal name exactly as it appears on your passport.
請填寫與護照一致的完整英文姓名。
2. Be sure to enter the full official English name of your school.
請填寫完整的官方英文或中文校名。
3. Please send a valid student ID or official school document to info@jgl.tw showing current enrollment.
請email有效的學生證或官方在學證明文件至info@jgl.tw。
4. This registration does not guarantee acceptance into any tournament. Additional application is required per event.
