ELIGIBILITY: Entries are open to amateur golfers who have an up-to-date USGA Handicap Index®. A Handicap Index must be issued from an IGA District I, II, or III member “golf club” (as defined in the “USGA Handicap System Manual”) which is licensed to use the USGA Handicap SystemTM. Or, for bona fide residents of the State of Idaho, Panhandle District, have an up-to-date USGA Handicap Index® issued from a WSGA licensed member “golf club.”
Players must have a current handicap index of 12.0 or less to participate in the Championship.
Players must be at least 25 years of age by the first round of the event.
All applications will be reviewed by the IGA to verify that each entrant has an official USGA Handicap Index® from an IGA member club or course.
Any amateur whose name does not appear on the IGA’s handicap roster will be ineligible and their entry application and entry fee will be returned.
ENTRY DEADLINE: All players must file an application by the entry deadline - Wednesday, August 28, 2019 11:00 p.m. MST to be eligible to compete in the Championship. Incomplete applications are not accepted. Entrants are urged to submit applications well in advance of the entry deadline.
THE IGA WILL NOT ACCEPT LATE ENTRY APPLICATIONS. FAXED APPLICATIONS ARE NOT ACCEPTED.
THE FIELD WILL FEATURE A MAXIMUM OF 75 TOTAL PARTICIPANTS
Any player within the following categories or on the wait list must have registered prior to the entry deadline. The following priority list will be used to determine entry into the championship field:
1. All past IGA Mid-Amateur Champions
2. Date of received entry into championship
ACCEPTANCE OF PLACES/ NOTICE OF WITHDRAWAL: A player who must withdraw before the championship must notify the IGA office (208) 342-4442 or the Official in Charge at the site of the event. If a player must withdraw during an event, he must notify the Official in Charge. If a player does not show for an event or fails to follow the above procedure, their actions may result in a one-year suspension from all IGA competitions. Said player forfeits all entry fees.
REFUNDS: No refunds for any reason (except for illness or injury) will be issued after 5:00 p.m. MST on the evening before the first round of the respective competition. Requests for a refund due to illness or injury will be treated on a case by case basis and can include illness or injury of a spouse or child. All refunds will be subject to a $25.00 withdrawal fee.
CANCELLATION/EXTENSION OF PLAY:
If a lengthy suspension of play occurs during the final round or is likely to affect the final round, the IGA will discuss options with the host facility on the cancellation of the final round and/ or shortening of the championship. For a match play formatted event, the original match play bracket will be completed. This may require the completion of a match play round(s) the day after the planned conclusion of the event. The IGA will discuss all of these options with the host club’s staff.

All payouts will be credited in the player’s name with the host club golf shop.
Entry Fees are $200 per Player (Plus 2.5% credit card processing fee of $5.00):
Entry fee includes: 2.5% credit card processing fee, player gift, practice round, two championship rounds (minimum), merchandise payout, and IGA championship trophies.
Wednesday, September 4: Practice Round, 18 holes
Thursday, September 5: First Round, 18 holes
Friday, September 6: Second Round, 18 holes
Saturday, September 7: Third Round, 18 holes
RULES OF GOLF: Play will be governed by the Rules of Golf. All questions shall be settled by the Committee in charge of the competition, whose decision is final.
Caddies are permitted for this Championship.
CONDUCT CODE: Penalties for breach of sportsmanship code (to include but not limited to property damage, abusive language, club throwing, excessive drinking or gambling) may include censure, suspension or revocation of playing privilege.
IGA DRESS CODE: Contestants must abide by the IGA dress code during practice and championship rounds. Shirts must have collars and sleeves and should be tucked in; tailored shorts or pants, no blue jeans.
Penalty for breach of IGA dress code - may be expelled from the Championship and/or suspended from future IGA championships.
FOOTWEAR: Traditional type spikes, such as metal, are prohibited at all practice rounds and at the championship rounds. Penalty for breach of this condition of play is disqualification.
By submission of this entry application, I agree to the following:
I verify that I have read the Championship Entry Form for this championship and agree to all of the rules and regulations attached therein. I have read and understand the requirements for acceptance of places, notice of withdrawal, refund of entry fee, handicap requirement, all eligibility requirements, and registration at the Championship. I agree that there are risks inherent in the game of golf, including injury, bodily harm, and death, and I agree to hold the Idaho Golf Association, its Officers, Directors, Staff, Volunteers, and all qualifying and championship host facilities/golf courses harmless from any claim whatsoever and accept personal and sole liability for all such risks and agree to indemnify the IGA from any claims as a result of injury to me. I agree that this entry application is subject to rejection at any time (including during the Championship) by the IGA. The reason for rejection may include conduct unbecoming of a tournament participant. I agree that the responsibility for ensuring this application is received by the IGA by no later than the published entry deadline is mine alone and that this entry application will be automatically rejected if received by the IGA after such date and time, regardless of the cause.
