2021 Arkansas Women's Stroke Play Championship

I understand that my entry is subject to rejection at any time including during the Championship by the ASGA or the host club. The reason for rejection may include unbecoming conduct. Any questions shall be settled by the ASGA, whose decision is final. I hereby acknowledge and agree that the Arkansas State Golf Association (the “ASGA”) and host club are taking commercially reasonable steps in accordance with Centers for Disease Control (“CDC”) guidelines and other applicable government regulations to provide a safe environment for the golf tournament being put on by the ASGA and the host club during the continuation of the ongoing Covid 19 pandemic.   By signature below or by completing the online entry and hitting the submit/enter button as described herein, I hereby acknowledge and agree that (i) I am fully aware and comprehend and appreciate the risks relating to the Covid 19 virus or other risks relating to the participation in this event, (ii) I understand and hereby voluntarily consent to accept these risks, and (iii) I hereby forever release, waive, acquit, and discharge the ASGA, the host club, and each of their respective owners, officers, employees, board of directors, agents, representatives, affiliates and assigns (collectively, the “Released Persons”) from and against all actions, rights, and causes of action, liabilities, damages, expenses, costs, claims and demands whatsoever that may exist or hereafter accrue against any Released Person who or which may be charged with responsibility for injuries to me, the treatment thereof, and all consequences flowing therefrom, as a result of participation in the golf tournament.

 

Eligibility: All players must be individual members of ASGA and full-time residents of the state of

Arkansas.  Non-residents are not eligible, with the following exception:  a person may live across the state line, such as Texarkana, Texas, but must be a member of the Texarkana Country Club and an individual ASGA member.  The minimum age is 16 unless approved by the ASGA Staff.

 

There are five (5) separate divisions:

 

CHAMPIONSHIP DIVISION 16+

Tees: Approximately 5,800+ Yards – FAYETTEVILLE C.C. – BLUE TEE

MID-AMATEUR 25+

Tees: 5,200 – 5,600 Yards – FAYETTEVILLE C.C. – WHITE TEE

SENIOR DIVISION 50+

Tees: 5,200 – 5,600 Yards – FAYETTEVILLE C.C. – WHITE TEE

SUPER SENIOR DIVISION 60+

Tees: Not to exceed 5,200 Yards – FAYETTEVILLE C.C. – RED/WHITE COMBO TEE

MASTERS DIVISION 70+

Tees: Not to exceed 5,200 Yards – FAYETTEVILLE C.C. – RED/WHITE COMBO TEE

 

Format: This event will be 36 holes held conducted at Fayetteville Country Club, with competitors playing 18 holes each day. Caddies will NOT be allowed this year, all spectators may be asked to walk due to a limited fleet of carts available.

 

Schedule of Events:

 

Tuesday, June 8 –Tee times begin at 8:00 a.m. Starting with the Championship Division and ending with the Masters Division. Lunch will be provided at Fayetteville Country Club for all competitors.

Wednesday, June 9 –Tee times begin at 8:00 a.m. Starting with the Masters Division and ending with the Championship Division. Lunch will be provided at Fayetteville Country Club for all competitors. Awards ceremony and prizes awarded immediately following the completion of play.

 

Prizes/Awards: All Play will be flighted following the first round within the division, based on field size. The low finishers in each flight will receive a gift certificate from the Fayetteville Country Club Pro Shop.

 

Entry Deadline – The application and entry fee must reach the ASGA office, P. O. Box 30250, Little Rock, AR 72260, no later than 4:00 p.m. Friday, May 28.  It is permissible to copy this entry form, if necessary.  Late or incomplete entries will not be accepted. Online registration is available at ASGA.org.

 

Practice Round – Practice Rounds are available for $70 per player including cart and range balls.  Tuesday – Thursday tee times will be available in accordance with FCC availability.  Friday – Sunday, tee times will be available after 2:00 p.m.   Players must call the FCC Golf Shop to schedule a practice round tee time. The weekend of June 3rd – 6th Practice Rounds are unavailable

 

Prizes – The Champion will receive custody of the S. W. Creekmore trophy plus pro shop merchandise.  Top finishers (approximately 1/3rd of field) will receive a gift certificate from Fayetteville Country Club.
