ENTRY PROCEDURE: Contestants shall apply on official OGA entry blank or online at www.ohiogolf.org. Entry fee refund available only for medical reasons after deadline date, September 5, 2022, 11:59 p.m. All refunds are subject to a $15 service fee. Late or incomplete applications not accepted. No telephone entries. On-line entry is available at WWW.OHIOGOLF.ORG.
APPLICANTS: Must be (1) a male (2) a resident of the State of Ohio. (3) have an active WHS Handicap Index at an OGA member club in order to pay the member fee. If one member of the team is a member of an OGA Member club with an active WHS Handicap Index, the member fee applies. A list of member clubs can be found at www.ohiogolf.org. A resident is defined as meeting four (4) of the nine (9) 1. Ownership or leasing of Ohio residence 2. Proof of payment of utilities at property for which residency is being claimed 3. Ohio homestead exemption declared on residence 4. Place of employment in Ohio 5. Ohio Driver’s License 6. Ohio Vehicle registration 7. Ohio address listed on federal income tax return filed by player 8. Has a WHS handicap at a golf club in Ohio. 9. Voter registration in Ohio.

85% of the player’s handicap index as of September 9 will be used for net divisions.
Worthington Hills CC Shop Credit awarded based on field size within division.
Winner of Overall Gross division is exempt for 2023, 2024 and 2025 Ohio Amateur.
REGISTRATION: All players are required to register at Worthington Hills CC prior to their first-round tee time on Monday, September 12, 2022.
PRACTICE ROUND: After entering, players may call Worthington Hills CC to arrange a Practice round between August 30th and September 11th based on availability.
614-885-9128. Practice round cost will be $50, cart may be additional. Availability may be restricted on September 10th and 11th, so plan accordingly.
CADDIES: A player must not have another player who has or will compete in the same Round as his or her caddie during the round. Penalty for breach of Local Rules: The player gets the general penalty for each hole during which he or she is helped by such a caddie. If the breach happens or continues between two holes, the player gets the general penalty for the next hole. Modern Local Rule 8H.
SHOES: During a round, a player must not wear shoes with
• Traditional spikes—that is, spikes having single or multiple points designed to penetrate deeply into the surface of the ground (regardless of whether made of metal, ceramic, plastic or other materials
• Spikes of any design that are entirely or partially made of meatal, If such metal may come in contact with the course

PAIRINGS: Pairings will be emailed to all contestants and posted to www.ohiogolf.org.
MEALS: Buffet Lunch included in entry fee Monday and Tuesday for contestants.
All entries shall be deemed to be by invitation and are subject to OGA Board acceptance.
