2024 ASGA Men's Four-Ball Championship

Eligibility - Players competing ni this Championship are required to be current individual members of the Arkansas State Golf Association and a current member of a club or golf association that is also a member. The two players are not required to be a member of the same club or golf association. ***NEW*** There will be a SENIOR Division open to the first 12 teams who register. The Field wil be limited ot the first 4 Amateur Teams and hte first 21 Senior teams. The minimum age si 18, unless approved by the ASGA.
Handicap Index - Both players must have a current ASGA-USGA Handicap Index. Total team handicap cannot exceed 15 strokes. Al scores will be gross scores.
Format and Starting Times - The format is 36 holes of four-ball stroke play. Tee times will be used for Saturday's round starting at 8:00 A.M.. Sunday's round will be a 9:00 a.m. shotgun start. Players may request Saturday's first round starting times. Requests wil be taken on a first come first serve basis and the ASGA wil try to accommodate as many players as possible. Both courses will be used for this championship - one played on Saturday and the other on Sunday. The rotation of courses will be posted at HSCC. Lunch will be served on Saturday; breakfast on Sunday.
Entry Deadline - The application and entry fee must reach the ASGA office, P. O. Box 30250, Little Rock, AR 72260, no later than 4:00 p.m. Monday, April 15. It is permissible to copy this entry form, fi necessary. Late or incomplete entries will not be
accepted.
Practice Round - Apractice round is by calling the HSCC Pro Shop at 501-624-2661 and reserving a tee time. Each player wil have to pay a reduced rate green fee ($76.00).
Prizes - The Champions will receive custody of the S. W. Creekmore trophy plus pro shop merchandise. Top finishers (approximately 1/3d of field) will receive a gift certificate from HSCC.
Iunderstand that my entry is subject to rejection at any time including during the Championship by the ASGA or the host club. The reason for rejection may include unbecoming conduct. Any questions shall be settled by the ASGA, whose decision si final. Ihereby acknowledge and agree that the Arkansas State Golf Association (the "ASGA") and host club are taking commercially reasonable steps in accordance with Centers for Disease Control
("CDC") guidelines and other applicable government regulations to provide a safe environment for the golf tournament being put on by the ASGA and the host club during the continuation of the ongoing Covid 19 pandemic. By signature below or by completing the online entry and hitting the submit/enter button as described herein, 1hereby acknowledge and agree that (i) Iam fuly aware and comprehend and appreciate the risks relating to the Covid 19 virus or other risks relating to the participation ni this event, (i) Iunderstand and hereby voluntarily consent to accept these risks, and
(i) | hereby forever release, waive, acquit, and discharge the ASGA, the host club, and each of their respective owners, officers, employees, board of directors, agents, representatives, affiliates and assigns (collectively, the "Released Persons") from and against al actions, rights, and causes of action, liabilities, damages, expenses, costs, claims and demands whatsoever that may exist or hereafter accrue against any Released Person who or which may be charged with responsibility for injuries to me, the treatment thereof, and al consequences flowing therefrom, as a result of participation ni the golf tournament.
