By submission of this entry application and participation in this Idaho Golf Association sponsored event, I agree to the following:

 

I verify that I have read the Championship Entry Form for this championship and agree to all of the rules and regulations attached therein.

 

I have read and understand the requirements for acceptance of places, notice of withdrawal, refund of entry fee, handicap requirement, all eligibility requirements, and

registration at the Championship.

 

I agree that there are risks inherent in the game of golf, including injury, bodily harm, and death, and I agree to hold the Idaho Golf Association, its Officers, Directors, Staff, Volunteers, and all qualifying and championship host facilities/golf courses harmless from any claim whatsoever and accept personal and sole liability for all such risks and agree to indemnify the IGA from any claims as a result of injury to me.

 

I also acknowledge that a risk of participating in the game of golf and interacting with other competitors or individuals could include the infection and transmission of the COVID 19 Virus and I agree to hold the Idaho Golf Association, its Officers, Directors, Staff, Volunteers, and all qualifying and championship host facilities/golf courses harmless from any claim whatsoever and accept personal and sole liability for all such risk and agree to indemnify the IGA from any claims as a result of infection or injury to me.  

 

I agree that this entry application is subject to rejection at any time (including during the Championship) by the IGA. The reason for rejection may include conduct unbecoming of a tournament participant.

 

I agree that the responsibility for ensuring this application is received by the IGA by no later than the published entry deadline is mine alone and that this entry application will be automatically rejected if received by the IGA after such date and time, regardless of the cause.
