2nd Annual Jack Shockley Peacemaker Open - Team Registration Form

Please follow the prompt to register your team for the 2nd Annual Jack Shockley Peacemaker Open 

 

 

Only ONE member from each team of four needs to submit this registration form. However, please add name & contact information for each member of the foursome by clicking the blue 'Add Another Player to my Team'' button in the bottom left corner of the form. 

 

 

Payment is required to complete the team registration form. The full team fee is $600 ($150/golfer) and provides each player with:

 

  • 18-hole scramble hosted at Prairie View GC on Friday, May 12th
  • Driving Range Access prior to the event
  • Sahm's Boxed Lunch
  • Player Gift Bags
  • Complimentary Beverage Carts
  • On-Course Prizes & Contests
